Annual Fees: $289 per adult patient and $249 per child (age up to 18)

Preventive Services Covered at 100%
- Two professional cleanings (ADA D1110 & D1120)
- Two exams with the doctor
- One set of necessary x-rays
- Two fluoride treatments (age up to 18)
- Ini0al New Patient exam and X-rays
- Oral hygiene and Home Care instructions
- Oral cancer screening
- Smile analysis and treatment planning
All other dental service fees reduced by 15% 
- Restorative filings, Extractions, Crowns, Veneers, Periodontics (gum), Root Canals, Implants, and Oral Surgery.
- Orthodontics (ClearCorrect invisible braces only) $500 off – member must remain a plan member for the dura0on of treatment.
Exclusions & Stipulations 
- Our membership is NOT a dental insurance plan, and can only be used at Lee Aesthetic Dentistry.
- It cannot be combined with any dental insurance coverage.
- Membership is not transferable, nor refundable.
- The 12 month membership fee is due in full upon joining. Payments are due when services are rendered.
- Yearly Renewal- Your credit card will be charged on the 1st day of anniversary month to ensure your continued coverage. A 30-day notice is required to opt-out of automatic renewal.
- It is the sole responsibility of the member to maximize their benefits by arranging all the appropriate appointments within the 12 month membership period. Unused benefits cannot be rolled over to the next year, or refunded.
- Membership rates may be reviewed and adjusted on an annual basis with notification.
